Measuring Quality and Performance in primary care

We are trying to understand the way different regions are evaluating the quality performance of the healthcare in the chronic diseases in the scope of this project. This also relate to "Outcomes That Matter".

We are currently in process to collect data from the 21 regions to understand which variables are utilized. These are then evaluated in in relation to their ability to provide a holistic answer to a decision maker in order to guide decision about funding and need for improvements.
